Click.GiftLoad Issue

I've been dealing with this for the last few days, generally computer savvy and know what I'm doing. I let my little brother on the computer and I got something like this.

I have tried to get this off several ways. I've done everything from OTL => Combofix and to no avail. However after I used OTL I was able to make this issue no longer appear on malwarebytes but it still shows up on Spybot.

AVG is showing a rootkit that's hidden and I didn't want to delete it since AVG didn't auto-kill it.

What do you need to see from me to help get this thing resolved?

I cannot reformat, It's not really an option. (If it comes down to it, I will need a day at least to reply)
