Unable to remove infection

It seems that i am unable to remove the following, even after rebooting it is still there any ideas

Malwarebytes is up to Version 1.42, did you try updating it?

And please note: You must remove all found malwares at the end of the scan

Also Windows Vista is up to Service Pack 2: http://support.microsoft.com/kb/948465

So that's 4 out of 4 you got wrong :(
