Win32k Stream removal

I've got an XP (SP3) Media Center machine that has had significant malware issues. I've been able to remove most with Stopzilla, but, as stated in other posts, I have been unsuccessful with the Win32K Stream trojan horse.

I've read numerous forums discussing this and each solution seems unique to the infection. Help would be greatly appreciated.
