Never before seen Malware (at least by me)

I have a Windows Vista, 64 bit system, that is driving me crazy. The system was badly infected with what apparently is a virus that my AVast could not catch. I installed and regsitered Malwarebytes on the system as well and it removed a bucn of stuff that AVast didnt' find ... but there are still 2 items that can't be removed... AVast finds them during a scan, but cannot remove them. I cannot do a boottime scan due to this being a 64 bit system so not sure where to go.

The files that AVast finds with every scan is:

c:\windows\temp\hki8186.exe win32:malware-gen and

c:\windows\temp\igrr.tmp\setup.exe win32:trojan.gen

Once the scan is complete, however, they are not there thus cannot be removed.

When running HiJackThis, there are a lot of lines that has "File Missing" that I cannot remove and I do not know how to proceed.

Anyone know how I can get rid of these items? (The Hijackthis log is below and attached in case attachments are better.)

Please don't attach the scan results, use Copy/Paste

DO NOT use any TOOLS such as Combofix or HijackThis fixes without supervision.

Doing so could make your pc inoperatible and could require a full reinstall of your OS, losing all your programs and data.

Vista and Windows 7 users:

1. These tools MUST be run from the executable. (.exe) every time you run them

2. With Admin Rights (Right click, choose "Run as Administrator")
