Help with removing AntiMalware Doctor

I have been trying for the past 2 days to remove anti malware doctor and nothing has worked. First, in normal mode, I downloaded malwarebytes but it didn't run, so I changed file name to explorer.exe and that worked and I was able to run it from program files. after the 2 hour scan, I removed all the items it detected (14).

then it said to reboot, which i did and right after restart i found that the antimalware doctor was still there because their scan popped up and asked me to remove threats. so i went into safe mode after rebooting again and scanned using malwarebytes and this time it found 1 item in the quick scan which i removed. then just to be sure, i ran the full scan which took about 2 hours and came up with nothing. still in the start menu in safe mode, i could see that there was a program called anti-malware doctor which was still there even after deleting all these files through malwarebytes, so i right-clicked on the antimalware doctor program in the start menu and clicked delete which removed it from the start menu, i felt satisfied enough so i rebooted and lo behold, there it was again and this time the computer took way too long to start up (in normal mode) and the desktop icons are in some old windows format (do not know why). (deleting AMD from add/remove programs is a trap which only leads to more of the malware)

so now what i did was do the rkill.com thing someone suggested on the cnet forums and that seemed to stop the antimalwaredoctor because i did not see it as an icon on my desktop taskbar (normally there are 2 icons for AMD, although the log from rkill did not mention that it had stopped any malware programs from running). so now i deleted the old malwarebytes, since i had installed it in the presence of the AMD (i'm going to abbreviate from hereon out) originally, and re-installed it (this took some trouble because the malware, which i thought was inactive from rkill, kept guiding me to false websites. finally i downloaded it again, did not change any file names because i wasn't sure which step in the installation i was supposed to so that it would have an effect on the AMD and i still cannot run malwarebytes. now i have no idea what to do. btw, in safe mode and safe mode w/ networking, i cannot connect to the internet and so i can't download malwarebytes in safe mode unfortunately. perhaps that is a solution? i just want my desktop to return back to the way it was before this mess and i am very exhausted after spending so many hours trying to get rid of AMD. I will really appreciate any help in regards to what I should do next.
