Things to keep in mind, as far as any situation with Microsoft Defender.

What is the name of the file ?

Where exactly is it located?

Is this about a item flagged today ?

Is it about Scan History ?

Is it about Quarantine History ?

Is it just about a Exclusion of a file or folder ?

I suggest that at minimum you do what follows & get the result posted here.

The Microsoft Safety Scanner is a free Microsoft stand-alone virus scanner that can be used to scan for & remove malware or potentially unwanted software from a system.

Let me know the result of this.

The log is named MSERT.log  

the log will be at  

C:\Windows\debug\msert.log

Please attach that log with your reply.
