I have performed a clean install of 3.5.1.2522 - Both using the clean install tool, and manually deleting ProgramFiles / ProgramData files followed by a reboot.

I am running Windows 7 x 64, 7601, up to date as of 6/3/2018.

Spec wise: Pentium E2180 / 2GB RAM.

What am I doing wrong / why is this button forever greyed out?

I have also tried to run SFC /scannow, and launching MWB as Admin to no avail.

You have compatibility mode turned on for Malwarebytes. Right click the Malwarebytes icon on your desktop, and choose Properties. Then go to the Compatibility tab on the top and make sure all the checkboxes are unchecked. Click Ok, and then close and open Malwarebytes again and you should be able to see Ransomware properly.

This is a side note and not related to the reported problem.

While a Windows 64bit OS will work under 2GB, it is not enough to make the OS efficient.  It is suggested to move 4GB or more.

You will notice a difference in performance with more RAM.

Hello! The Ransomware Protection is not enabled.

Fun fact! It WAS on compatibility mode. What a PITA.

Also, I did upgrade the system to 4GB while the posts were waiting. I've only got 2 DIMM slots and an Intel Chipset, so that's as good as it gets.

Cheers lads.
