It could be the AV blocking the installer for the new version itself, as it would be impossible to have it added to the trusted zone since it gets downloaded and executed from a temp location. I know Kaspersky always blocks some features (the drivers) from installing properly unless I tell it to ignore the setup file for a new version as it starts to execute. If your AV doesn't give a warning but instead just blocks it by default then the only solution would be to temporarily disable the AV while MBAM's new version installs.
